Support for Patients

Support for Patients with Learning Disabilities

The Trust has a Learning Disability Liaison Nurse for advice or support and they can be contacted on (01983) 822099 Ext 5478 or Mobile 07795 121674.

The nurse can:

  • Make sure the patient has access to all the information needed when coming into hospital
  • Inform hospital staff of any special arrangements required for the patients appointment, treatment or stay in hospital
  • Support the patient in understanding and communicating their needs
  • Co-ordinate discharge arrangements between the patient, carers, community and hospital staff

Support for Patients with Memory Loss

If you are concerned about your memory or that of a loved one, we have qualified staff that are available for confidential advice and support. 

We can help you with: 

  • Support and advice for carers
  • Promoting wellbeing, maintaining existing cognitive functioning and providing excellent quality of life for people with all types and stages of dementia
  • Identifying local support groups, clubs and social events for people with dementia and their carers or families
  • Getting access to facilities and care options that are available for a wide range of needs and requirements across the island
  • Accessing local and national agencies that can offer a comprehensive range of advice and support for all aspects of dementia care 

We can also signpost you to other agencies which may be able to help such as Age Concern, The Alzheimer’s Society and many others.
