‘My Life: A Full Life’ is a collaboration between the newly established Clinical Commissioning Group, NHS Trust and the Isle of Wight Council. The new initiative will work in partnership with local people, voluntary organizations and the private sector to deliver a more coordinated approach to the delivery of health and social care services for older people and people with long term conditions on the Island. Further information can be found on the ‘My Life: A Full Life’ page.
