You can use this portal to request the record that we hold for you and that is in line with GDPR and local retention policies.
Requests made via this portal will only return reports on the digital evidence that we hold in OH and where we have undertaken and administered the vaccine or blood test
This portal will not provide reports or evidence of vaccinations and blood tests that were administered elsewhere, even if you have shared paper or digital copies with our team for them to review at any stage. For example, for vaccinations given by your GP or another healthcare provider please contact them directly for reports and assistance.

Were you offered OH clearance before 2022?
If the answer is YES then please visit our department during office hours, ensuring you bring appropriate and up to date I.D and speak with one of our reception team. They will be able to review records relating to paper formats and previous OH databases and inform you with what is held in line with GDPR and local retention policies.
Requests made via our reception team will only return immunisation evidence in cases where we have undertaken and administered the vaccine or blood test.
Our reception team will not be able to provide reports or evidence of vaccinations and blood tests that were administered elsewhere, even if you have shared paper or digital copies with our team for them to review at any stage. For example, for vaccinations given by your GP or another healthcare provider please contact them directly for reports and assistance.
Please note that waiting times will vary when visiting our department due to the work demands and priorities on the day. Therefore, please allow yourself plenty of time when visiting our department and for our reception team to assist you
If you are unable to attend our department then please email your request to iownt.occupationalhealth@nhs.net with your full details. Our team will review records relating to paper formats and previous OH databases held in line with GDPR and local retention policies. Reports will then be made available for you to collect within 28 days. Upon collection you must ensure you bring appropriate and up to date I.D and speak with one of our reception team to assist you further.
Should you be unable to attend our department to collect your report or certificate and you request for us to email it to you instead, then please be advised that we will be happy to send this to an active NHS email account.
If you have never been offered OH clearance?
Please contact HR Recruitment teams and your line manager for them to clarify and confirm what clearance level has been offered for your role
If a clearance level is known, then please follow the appropriate guidance above
If a clearance level is unknown, then please be aware there may be times where staff are given start dates and/or offered shifts before the OH processes have been completed. An example may be that you started your role under a temporary risk assessment carried out by your line manager. In these cases, there is a possibility that we do not have accurate records or that vaccinations and blood tests are outstanding.
Therefore, in order for us to provide any form of record or certificate detailing immunisations we have administered we advise you contact the HR recruitment team and request they add or update the OPAS G2 record that will allow us to review and provide appropriate clearances and reports

Immunisation Identity Validated Requests
Where applicable, all reports and certificates requested for Identity Validated Samples (IVS) should be made in writing to iownt.occupationalhealth@nhs.net where one of our clinical team will provide further signed documentation at the earliest opportunity. Requests for IVS reports and certificates may take up to 28 days to process.
