Children in Care Health Team
Recent changes to the Children in Care ( CiC) health service provision mean we now only accommodate Initial Health Assessments, Review health Assessments and adoption medical requests from the IOW Local Authority and Mainland Local Authority for children in care placed on the IOW. Due to the service changes, we are unable to respond to requests for follow up support, including visits which involves direct work with the children and young people open to our service outside of the assessment process, and we can no longer offer Care Leaver support services
The service delivers direct clinical contact, advice and support within the health assessment process to Children in Care, their Carers and Social Workers to address health issues unique to this group of Children and Young People. The team is part of the Integrated Safeguarding Team within the IOW NHS Trust
Our Children in Care (CiC) team consists of:
We work closely in partnership with other Health, Social Care and Education to help CiC/Care experienced children and young people, to remain healthy both physically and emotionally. The team arrange, co-ordinate and undertake Statutory Health Assessments in accordance with Department of Health Statutory Guidance.
Initial Health Assessments (IHA) are completed when a child first comes into care by a registered medical practitioner. Review Health Assessments (RHA) coordinated by the Children In Care & Care Leavers Nursing Team and will be undertaken either by practitioners from the Solent NHS 0–19 Public Health Nurse Service OR by our Children in Care Team.
Review Health Assessments will be undertaken
Health Promotion is provided at health assessments and on contact with carers or children - information such as Healthy Eating, Dental Care, Alcohol, Substance Misuse, Sexual Health, Healthy Lifestyles, Smoking Cessation, Safe Relationships and Emotional Wellbeing.
