Information For GPs

Referrals can be made through the electronic referral system. The chronic pain proforma will need to be completed as part of this referral.

The electronic referral will be triaged and checked. The referral will then, either be sent back for clarification, refused or placed on hold until the pain questionnaire is returned by the patient. The chronic pain team will send the questionnaire to the patient.
